A 45-year-old male suffered from subarachnoid hemorrhage. Cerebral angiography showed an aneurysm in the anterior communicating artery. He underwent successful surgery 3 days after onset. The aneurysm was clipped after clipping the anterior communicating artery for 10 minutes. He noticed visual disturbance 2 days after surgery. The visual acuity was 1.2 right and no light perception left. There were no abnormalities in the eye position, ocular motility and convergence. The left eye showed absence of direct light reaction and presence of indirect reaction manifesting Marcus-Gunn pupil. Computed tomography showed normal findings in the optic nerve. Cerebral angiography showed patent ophthalmic artery. Fundus-copy showed normal findings in the central retinal artery and vein. Fluorescein angiography showed normal arm-to-retina circulation time and normal retinal vessels. Intraoperative compression of left ophthalmic artery was suspected as one of the causes of the impaired optic nerve.
