We modified surgical approach to macular holes. The method consists of pars plana vitrectomy, removal of the cortical vitreous, suction or scraping of retinal pigment epithelium through the macular hole and air tamponade. We applied this method to 16 eyes with full-thickness macular hole, including 11 eyes in stage 3 and 4 eyes in stage 4. Visual impairment had persisted for 1 to 120 months, average 22 months. The postsurgical follow-up ranged from 2 to 10 months, average 5.2 months. Closure of macular hole with resolution of subretinal fluid was attained in 14 eyes, 88%. The visual acuity improved by 2 or more lines in 8 eyes, 50%. The final visual acuity in 14 eyes with closure of the hole was 0.7 or better in 5 eyes, 36%, and 0.3 or better in 9 eyes, 64%.
