自家血清を応用した全層黄斑円孔の硝子体手術

Vitreous surgery with autologous serum for full-thickness macular holes

　Gass分類Stage 4の2症例に自家血清を応用した硝子体手術を行った。2例とも術後円孔の閉鎖が得られ，視力も2段階以上向上した。しかし，1例で術後2日目に眼底にフィブリンが索状に析出し，鼻側に牽強性網膜剥離が観察された。術中特に併発症が認められなかったことから，この反応に自家血清が関与している可能性が示唆された。黄斑円孔に対する自家血清を応用した硝子体手術が有用である可能性が示唆されたが，その本格的応用にはさらなる研究が必要であると思われた。

We treated two eyes of full-thickness macular hole by pars plana surgery. After filling the vitreous cavity by air, a drop of autologous serum was placed onto the macular hole. The serum was removed after 5 minutes and the vitreous was filled by SF₆₎ gas. The macular hole was anatomically closed and the visual acuity improved by 2 lines or more. On day 2 of surgery, traction retinal detachment developed in one eye with fibrin formation in the nasal inferior quadrant. Since the surgery was free of problems, this complication appeared to be related with the use of autologous serum.
