Abstract Purpose:To report the characteristics of spontaneously closed full thickness macular hole(MH).

Design:Retrospective observational case series.

Methods:Eleven eyes of 11 patients were studied. Traumatic and post-vitrectomy MHs were excluded. The measured outcomes included images of spectral domain optical coherence tomography(SD-OCT), best corrected visual acuity(BCVA)for six months after spontaneous closure of MH, and the duration of spontaneous closure.

Result:Mean MH aperture diameter was 87.5±48 μm(range 22〜159 μm). Vitreomacular traction(VMT)was observed in 4 eyes(36%), in all of them MHs closed after release of VMT. Four eyes without VMT(36%)had epiretinal membrane. The growth of retinal tissue bridging over MH was observed in one eye. The duration of spontaneous closure ranged from 2 to 15 weeks. All eyes demonstrated continuity of both outer nuclear layer and external limiting membrane at MH closure. Ellipsoid zone was restored in 6 eyes at 2 months after MH closure. BCVA improved immediately after spontaneous closure(p<0.01).

Conclusion:Spontaneous closure of MH seems to depend on a small hole diameter. Release of VMT is not necessary to spontaneous closure of MH. Shrinkage of ERM and bridging formation are also important factors to facilitate spontaneous closure.
