鼻腔・副鼻腔悪性腫瘍摘出術と同時に施行した鼻涙管シリコーンチューブ挿入術

Silicone intubation into the nasolacrimal duct during surgery for malignant tumors in the nasal cavity and/or paranasal sinus

　上顎洞部の扁平上皮癌と，鼻腔・篩骨胞巣・上顎洞の悪性黒色腫の各1例に根治手術を行い，同時に鼻涙管にシリコーン管を挿入した。シリコーン管挿入は，本来の鼻涙管の断端の内腔を維持することが目的であった。両症例で術後の鼻涙管閉塞を防ぐことができ，良好な経過をたどった。流涙や涙の停滞などの合併症を予防でき，患者の術後のQOLの向上に寄与した。本法は侵襲が小さく，鼻腔または副鼻腔腫瘍の術後の鼻涙管閉塞を防止するのに有用であると結論される。

We performed radical surgery in two patients. One had squamous cell carcinoma of the maxillary sinus. The other had malignant melanoma in the nasal cavity, ethmoidal sinus, and maxillary sinus. A silicone tube was in-serted into the nasolacrimal duct during surgery. By this procedure, the lower end of the lacrimal duct was kept open in both cases without lacrimation or discharge after surgery. Direct silicone intubation thus contributed to improve the quality of life of patients. This procedure is almost free of insult and is recommended to prevent nasolacrimal duct ob-struction following surgery for malignancies in the nasal cavity or paranasal sinus.
