We reviewed the incidence and progression of retinal detachment between 1995 to 2000 in Goto archipel-ago in Nagasaki Prefecture. Rhegmatogenous retinal detachment occurred in 39 eyes of 38 patients. The incidence was one patient among 11,932 inhabitants. The series comprised 14 males and 24 females, of which 84% were aged 50 years or over. Their ages averaged 61.9 years. There were 29 eyes (74%) of rhegmatogenous retinal detachment and 10 eyes (26%) of macular hole retinal detachment. All the patients were referred to institutions in the mainland. Dur-ing the interval of referral, the visual acuity deteriorated further in 10 patients (26%). This feature was more apprent in patients resident in less inhabited areas. The observed findings seemed to reflect the age distribution and environ-ment peculiar to remote islands.
