　33歳女性の左眼に，耳側視野欠損が1週間前に突発した。矯正視力は両眼とも1.5で，眼底には検眼鏡的に異常がなかった。左眼視野に盲点拡大があり，multifocal ERG （MERG）で左眼乳頭を含む領域に応答密度が低下していた。急性特発性盲点拡大症候群acute idiopathic blind spot enlargement　syndrome （AIBSE）と診断し，プレドニゾロンの全身投与を開始した。発症の2週間後に盲点が縮小し，自覚症状が改善したが，MERGの応答密度には変化がなかった。本症候群の網月莫機能検査としてMERGが有用であった。

A 33-year-old female presented with left visual field defect since one week before. Her corrected visual acuity was 1.5 in either eye. Funduscopic findings were inconspicuous. Perimetry showed enlarged blind spot in the left eye. Multifocal electroretinogram (MERG) showed decreased amplitude around the left optic disc. She was diag-nosed with acute idiopathic blind spot enlargement syndrome (AIBSE) and was given systemic prednisolone. The en-larged blind spot and subjective symptoms improved 2 weeks after onset. Findings by MERG remained unchanged. This case illustrates the usefulness of MERG in detecting subclinical retinal dysfunction in this syndrome.
