成人T細胞白血病に続発したサイトメガロウイルス網膜炎の1例

Cytomegalovirus retinitis in a case of adult T-cell leukemia

要約　46歳女性が左眼霧視で受診した。1年前に成人T細胞白血病およびB型肝硬変と診断され，化学療法を受けていた。矯正視力は右0.8，左0.04であった。血管に沿う滲出斑と網膜出血が両眼の眼底にあった。サイトメガロウイルス（CMV）抗原陽性であり，サイトメガロウイルス網膜炎と診断した。ガンシクロビル点滴投与を開始し，その3週後に眼底所見が改善し，CMV抗原が陰性化した。成人T細胞白血病と化学療法による免疫抑制状態がサイトメガロウイルス網膜炎の誘因になったと推定される。

Abstract.　A 46-year-old female presented with blurring of vision in her left eye. She had been diagnosed with adult T-cell leukemia and hepatitis B with cirrhosis one year before. She had been receiving chemotherapy. Her corrected visual acuity was 0.8 right and 0.04 left. Funduscopy showed paravascular exudates with hemorrhage in both eyes. She was positive for cytomegalovirus（CMV）antigen. These findings led to the diagnosis of CMV retinitis. Fundus findings improved and CMV antigen became negative 3 weeks after start of intravenous ganciclovir. CMV retinitis appeared to be due to immunosuppressed state secondary to adult T-call leukemia and chemotherapy.
