We performed surgery for traumatic cataract in the right eye of a 56-year-old female. The cataract was presumably caused by perforating injury due to an arrow 42 years before. Her visual acuity was light perception right and 1.2 left. The right eye showed a scarred corneal wound at the 12 o'clock position, a small pupil, and organized cataract. We performed an iridotomy at the site of anterior synechiae, removal of the lens together with the organized lens capsule, excision of opaque vitreous, intraocular lens insertion and pupilloplasty. The visual acuity improved to 0.3. This case illustrates that surgery may be effective for traumatic cataract persistent over decades.
