Abstract Purpose: To report a case of traumatic cataract with formation of fibrous tissue that necessitated particular care in cataract surgery.

Case: A 45-year-old male presented with impaired vision in the left eye since a few years before. He had been injured in the left eye by a tip of a pencil at the age of 8 years.

Findings and Clinical Course: Corrected visual acuity was 1.5 the right eye and counting fingers at 30 cm in the left. The left cornea showed opacity in the lower nasal quadrant, with iris defect and lens opacity in the same area. Cataract surgery showed fibrous tissue formation bridging the corneal opacity and the lens. The intraocular lens was fixated by placing one of the supporting loops on the fibrous tissue. There has been no dislocation of intraocular lens for one year after surgery with corrected visual acuity of 1.0.

Conclusion: Formation of fibrous tissue bridging the lens and cornea necessitated particular care in surgery of traumatic cataract.
