高度近視眼の深部沿血管裂孔から生じた網膜剥離の3例

Retinal detachment with paravascular microholes in the posterior retina in three cases of high myopia

　後部強膜ぶどう腫を有する高度近視眼で，深部大血管近傍の裂孔から生じた網膜剥離の3例3眼を経験した。全例に硝子体手術を行い，硝子体手術中の共通の所見として，後部強膜ぶどう腫内および近傍の網膜血管周囲に強い硝子体網膜癒着を認めた。硝子体収縮に伴う牽引が加わることによって，裂孔が形成され網膜剥離を生じるものと考えられる。このような症例では，硝子体手術による確実な硝子体網膜牽引の除去が効果的と考える。

We observed retinal detachment in 3 eyes of high myopia with posterior staphyloma. Microholes were present adjacent to major retinal vessels in the posterior retina. Vitreous surgery induced retinal reattach-ment in all the 3 eyes. As intraoperative finding, a firm vitreoretinal adhesion was present within the staphyloma and in the area of paravascular microholes. It appeared that traction to the retina secondary to vitreous contraction would have induced the formation of microholes. Vitreous surgery is advocated to remove the vitreoretinal traction and to induce retinal reattachnent.
