超音波乳化吸引，眼内レンズ挿入術のベーチェット病の眼発作に影響する因子

Risk for ocular attacks following phacoemulsification and intraocular lens implantation in patients of Behçet disease

　ベーチェット病の併発白内障に対して，超音波乳化吸引と眼内レンズ挿入術を施行した12例16眼について検討した。最終発作から手術までの期間が6か月未満の症例や，罹病期間が長い症例で，術後1年間に眼発作を生じる割合が有意に高くなった。全身症状や内服薬などは，術後の眼発作との関連はなかった。手術を契機とした眼発作頻度の増加はみられなかった。術後は全例で視力改善が得られ，重篤な術中・術後合併症は認められなかった。ベーチェット病患者に対しても比較的安全な白内障手術が可能であり，手術時期は最終発作から6か月以上の期間をおくのが望ましいと考えられた。

We reviewed 16 eyes of 12 patients with Behçet disease who underwent intraocular lens implantation for complicated cataract. The incidence of acute ocular attack during the first postoperative year was significantly higher in longstanding cases and in patients who received surgery within 6 months after the last attack. The incidence of acute ocular attack was independent on the extraocular manifestations or sysemic medication Cataract surgery did not induce more frequent ocular attacks. Postoperative visual acuity improved in all the eyes. Severe intra- or postoperative complications were absent throughout. The findings show that cataract surgery in patients of Behçet disease is relatively safe when performed 6 months or more after the last ocular attack.
