視神経網膜炎を伴った猫ひっかき病の1例

Neuroretinitis in a case of cat-scratch disease

　猫4匹を飼育している17歳女性に発熱と頭痛が突発した。第8病日に左側頸部リンパ節が有痛性に腫脹した。第26病日に左眼に霧視が生じ，第39病日に当科を受診した。矯正視力は右1.2，左0.3であった。左眼に盲点の拡大と視野狭窄があった。左眼底に視神経乳頭の発赤腫脹，漿液性網膜剥離，白色滲出斑の散在があった。Bartonella henselaeの血清抗体値の上昇があり，猫ひっかき病と診断した。プレドニゾロンの全身投与を行い，眼底所見は軽快し，第63病日には左眼視力は1.0に改善した。網脈絡膜瘢痕と視野狭窄が残った。猫ひっかき病による網脈絡膜炎の予後は良好とされているが，視野狭窄が持続する事例があることを本症例は示している。

A 17-year-old female developed fever and headache. She had four cats in her house. On day 8, she developed painful lymph node swelling in the left neck. Blurring of left eye occurred on day 26. She was seen by us on day 39. Her corrected visual acuity was 1.2 right and 0.3 left. Enlarged blind spot with visual field constriction was present in her left eye. Funduscopy showed hyperemic disc swelling, serous retinal detachment and disseminated exudates in the left eye. She was diagnosed as cat-scratch disease due to elevated serum antibody titer for Bartonella henselae. Systemic prednisolone resulted in improved fundus findings. The left visual acuity recovered to 1.0 on day 63 in spite of chorioretinal atrophic lesions and visual field constriction. This case illustrates that neuroretinitis secondary to cat-scratch disease may run a benign course but that visual field constriction may persist.
