角膜内皮細胞異常を伴った瞳孔膜遺残の1例

A case of bilateral persistent pupillary membrane with reduced corneal endothelial cell population

　71歳女性が視力低下で受診した。両眼に老人性白内障と瞳孔膜遺残があった。角膜内皮細胞数は，1mm²あたり右1,251，左895であった。超音波乳化吸引術と眼内レンズ挿入術を行い，経過は良好であった。手術時に採取した瞳孔膜にはメラニン細胞が多数あり，壁が肥厚した血管を含んでいた。瞳孔膜遺残と角膜内皮は，発生的に神経堤由来であるので，瞳孔膜遺残があるときには，内眼手術を行う際に角膜内皮にも注意する必要がある。

A 71-year-old female presented with decreasing vision. Both eyes showed senile cataract and persistent pupillary membrane. The corneal endothelial cell population was 1,251 cells/mm² right and 895 left. She underwent uneventful phacoemulsification-aspiration with intraocular lens implantation. The surgically obtained pupillary memrbane showed numerous melanocytes and thick-walled blood vessels. Because of the disorder of neurocrest would lead to both persistent pupillary membrane and the corneal endothelium, due precaution is advocated in planning intraocular surgery in such cases.
