散瞳薬による開放隅角緑内障の眼圧上昇

Elevation of intraocular pressure induced by mydriatics in open angle glaucoma

（P−1-43）　開放隅角緑内障のトロピカミド，フェニレフリン合剤（ミドリンP^®）点眼後の眼圧変化につき調査した。点眼1時間後の眼圧上昇は開放隅角緑内障眼2.6±2.7mmHg，嚢性緑内障眼2.7±2.0mmHg，偽落屑（PE）のない非緑内障眼0.1±1.5mmHg，PEのある非緑内障眼0.5±1.6mmHgで，開放隅角緑内障眼嚢性緑内障眼では有意に眼圧上昇をきたした（p＜0.0001）（student-t検定）。4mmHg以上の眼圧上昇をきたした例は開放隅角緑内障40眼中14眼（35％），嚢性緑内障16眼中6眼（38％）と高率であった。開放隅角緑内障，嚢性緑内障において，散瞳検査を要する場合は注意が必要であり，最初に点眼後の眼圧測定をしておくべきだと思われた。また眼圧上昇例では，より注意深い経過観察を要すると思われた。

We investigated the changes in intraocular pressure (IOP) following instillation of Mydrin^TM which contains 0.5% tropicamide and 5% phenilephrine. The tested eyes comprised open-angle glaucoma 40 eyes, capsular glucoma 16 eyes, normaltension glaucoma 35 eyes and 434 eyes without glaucoma. One hour after two instillations of mydriatics with an interval of 5 minutes, the TOP rose by an average of 2.6±2.7 mmHg in POAG, 2.7±2.0 mmHg in capsular glaucoma, and 0.1±1.5 mmHg in eyes without glaucoma. The IOP elevation was significant in open-angle and capsular glaucoma (P＜0.0001 each, student t-test). Four glaucoma eyes (35%) and 6 capsular glaucoma eyes showed IOP elevation by 4 mmHg or greater. The findings show that due precaution is necessary in attempting mydriasis in open-angle or capsular glaucoma eyes.
