 We performed vitreous surgery on 30 eyes of central retinal vein occlusion. All the eyes had clinically significant macular edema. The outcome was evaluated at least one year after surgery. The series was divided into 9 eyes of nonischemic and 21 eyes of ischemic group based on final findings of fluorescein angiography. In nonischemic group,the median visual acuity improved from the preoperative 0.22 to postoperative 0.93. The difference was significant(p<0.005). In ischemic group,the median visual acuity remained the same at 0.07 preoperatively and 0.09 postoperatively. In these 21 eyes,the final visual acuity was the same or better in 16 eyes(76%). Rubeosis iridis developed in one eye as the sole major complication. The findings show that vitreous surgery may be effective when persistent macular edema is the major cause of visual impairment in central retinal vein occlusion.
