Visual field loss is a newly recognized complication of vitreous surgery. The flow volume of air was measured to estimate the adverse effect of air/fluid exchange during vitreous surgery. Dynamics of air flow was also checked by a model system. The flow volume of air during air/fluid exchange was found between 0 and 420 ml/min. The air flew straight for 30 mm or more from the opening of the irrigation needle, suggesting that the retinal area facing the irrigation port was subjected to the direct effect of inflowing air. This finding shows the importance to minimize the flow volume of air during air/fluid exchange, including the regulating system in the vitrectomy instrument inclusive the irrigation cannula, and an intraoperative monitoring of the intraocular environment.
