アトピー性皮膚炎に伴う硝子体基底部裂孔に対する経結膜冷凍凝固術の効果

Effect of transconjunctival cryoretinopexy for retinal breaks located in the vitreous base associated with atopic dermatitis

（B2-2-32）　過去17年間に，アトピー性皮膚炎に併発した硝子体基底部の網膜裂孔14眼に対して経結膜冷凍凝固術を行った。網膜剥離は，ないか，またはごく狭い範囲に限局していた。網膜裂孔は12眼（86％）で瘢痕化し，平均26か月の観察期間中，網膜剥離は拡大しなかった。アトピー性皮膚炎に併発した硝子体基底部の網膜裂孔には，網膜剥離があっても剥離部全体に冷凍凝固を加えれば剥離を拡大させない効果があると結論される。アトピー性皮膚炎では他の部位に裂孔が生じることがあるので，眼部の自己打撲をしないよう指導すべきである。

We treated 14 eyes in 12 patients of atopic dermatitis with retinal breaks located in the vitreous base by ransconjunctival cryoretinopexy during the past 17 years. Retinal detachment was either absent or was localized. During the follow-up averaging 26 months, the retinal break become scarred without enlargement of retinal detachment in 12 eyes (86%). The finding seems to show that cryoretinopexy is effective for retinal breaks located in the vitreous base in atopic patients when the whole area of retinal detachment, if present, is also cryocoagulated. We also advocate to advise the patients against rubbing or tapping the eye to prevent new retinal break formation.
