Candida albicansによる両眼性角膜真菌症の非定型例

A case of atypical bilateral keratomycosis by Candida albicans

（18-P2-37）　非定型的な所見を呈した両眼性のCandida albicansによる角膜真菌症の1例を報告した。症例は76歳女性で，両眼角膜の比較的大きな上皮欠損部に多数の白色隆起性病変がみられたが，毛様充血や細胞浸潤などの炎症反応はみられなかった。角膜ジストロフィなどの非感染性疾患が考えられたが，角膜擦過標本の組織診にて真菌要素が認められ，培養にてC．albicansが同定された。フルコナゾールの点眼・内服などの治療により，右眼は癒着性白斑，左眼はパンヌスを伴った角膜白斑の状態で治癒した。本例はベタメタゾン点眼が使用されていたため，炎症反応を欠く非定型的な所見を呈したものと思われた。

A 76-year-old female presented with bilateral corneal erosion with multiple white elevated lesions of 8 weeks' duration. Ciliary injection or cell infiltrations were absent. Biopsy of scraped corneal lesion showed fungus organisms which proved to be Candida albicans after culture. Treatment with oral and topical antifungal agents including fluconazole resulted in adherent leucoma in the right eye and leukoma with pannus in the left. This case of atypical keratomycosis without inflammation appeared to be due to prior treatment with betamethasone.
