痒感と異物感を主訴とする70歳の女性2名が両眼性の点状表層角膜炎と診断された。両症例とも通年性のアレルギー性結膜炎があり,クロモリンまたはケトチフェンを点眼していた。1例には涙液分泌減少症のために生理的食塩水の点眼を行っていた。1日4回のトラニラスト点眼を開始し,その約3週後に角膜炎は改善傾向を示し,約6週間でほぼ消失した。奏効機序として,ケミカルメディエータ遊離などのアレルギー反応が抑えられたことと,角膜の創傷治癒が促進されたことが推定された。
Two 70-year-old females presented with bilateral superficial punctate keratitis. Itching and foreign body sensation were cardinal complaints. Both cases had been suffering from perennial allergic conjunctivitis and had been treated by topical cromolyn or ketotifen. One case had been treated by topical saline for persistent hypolacrimia. We treated both cases by instillation of tranilast ophthalmic solution 4 times daily. The keratitis showed signs of improvement after 3 weeks and disappeared after about 6 weeks. The beneficial effect appeared to be due to inhibition of allergic reactions including release of chemical mediators and to acceleration of wound healing through suppression of corneal irritation.
