A 23-year-old male developed acute progressive loss of vision since one week before. When seen by us, the visual acuity was nil in either eye. The pupillary reflexes were abolished. He was tentatively diagnosed as acute retrobulbar neuritis. The patient confessed, next day, to be a habitual sniffer of organic solvent, or thinner, of eight years'duration. Analysis of the thinner showed 80% methanol as its main component. He was then treated by hyperbaric oxygenation, massive systemic corticosteroid and systemic vitamin B1 and B12. The visual acuity recovered to 0.05 right and 0.04 left with optic atrophy 7 months later.
