メチルアルコールガス中毒による急性球後視神経炎の症例

A case of acute retrobulbar neuritis by methanol gas inhalation

　シンナーは種々の有機溶剤の混合剤である。今回，シンナー長期吸引歴のある23歳の男性が，1週間で急激に両眼視力を喪失した症例を経験した。患者は両眼の視力喪失で来院し，光覚弁なく，瞳孔は散大し，対光反射は消失していた。初診時，シンナー吸引歴がわからなかったので，急性球後視神経炎ないし詐病が疑われた。翌日，シンナー濫用者と判明し，吸引していたシンナーの内容分析により気化状態でメチルアルコールが約80％と高濃度に検出された。メチルアルコールガスの長期吸引による慢性中毒が急性増悪し，急性球後視神経炎を発病したと診断し，高圧酸素療法，ステロイド大量投与とビタミンB1,B12，投与を行った。7か月後，両眼に視神経乳頭の萎縮を残したが視力は右眼0.05（矯正不能），左眼0.04（矯正不能）まで回復した。

A 23-year-old male developed acute progressive loss of vision since one week before. When seen by us, the visual acuity was nil in either eye. The pupillary reflexes were abolished. He was tentatively diagnosed as acute retrobulbar neuritis. The patient confessed, next day, to be a habitual sniffer of organic solvent, or thinner, of eight years'duration. Analysis of the thinner showed 80% methanol as its main component. He was then treated by hyperbaric oxygenation, massive systemic corticosteroid and systemic vitamin B1 and B12. The visual acuity recovered to 0.05 right and 0.04 left with optic atrophy 7 months later.
