A 31-year-old woman at 32 weeks'gestation presented with right homonymous hemianopia in both eyes. She had also upper respiratory infection. Her right eye showed relative afferent pupillary defect. The findings were consistent with left optic tract syndrome. T2-weighted magnetic resonance imaging showed T2 prolongation involving the left optic tract and thalamus, suggesting demyelination. Cerebrospinal fluid showed mild lymphocytosis and positivity of myelin basic protein. These findings led to the diagnosis of acute disseminated encephalomyelitis (ADEM). Intravenous high-dose corticosteroid failed to influence the hemianopia. To our knowledge, this is the first report of ADEM during pregnancy.
