眼循環障害における頸動脈病変のカラードプラ法による検出

Detection of carotid artery disorders underlying ocular vascular lesions by color Doppler sonography

　眼の循環障害に起因する眼疾患52例を対象にカラードプラ法で頸動脈病変の有無とその程度を調べた。52例中50例で頸動脈の描出が可能であり，このうち20例（40％）に頸動脈に病変がみられた。この20例中6例では罹患眼と同側の内頸動脈閉塞または狭窄がみられ，その眼疾患は網膜動脈閉塞症，一過性黒内障，および網膜静脈閉塞症であった。したがってこれらの眼疾患をみた時には，全身的に無症状であっても，外科的に治療しうる頸動脈病変のスクリーニング法としてカラードプラ法は有用であると思われた。

We performed color Doppler sonography to detect carotid artery disorders in 52 patients with ocular vascular symptoms. Abnormality in the carotid arteries were detected in 20 cases including 6 cases of severe stenosis or obstruction in the ipsilateral internal carotid artery. Cases of retinal artery occlusion and amaurosis fugax showed high incidences of carotid disorders at 30% and 20% respectively. Screening of patients with ocular vascular disorders by doppler sonography for carotid disorders is recommended as some of the carotid lesions can be treated by surgical means.
