We evaluated the anti-inflammatory effect of topical dexamethasone and diclofenac in 22 eyes after cataract surgery by phacoemulsification and posterior lens insertion. Laser flare cell meter and specular microscope were used in the evaluation. The aqueous flare value was the same for dexamethasone and diclofenac and wasthe lowest for both agents combined. The mean cell area of corneal epithelium showed no change after dexamethasone. It increased with diclofenac and with both agents combined after surgery and returned to the initial value 3 months later. The corneal epithelial score increased at two weeks after surgery and was the lowest with dexamethasone. The anti-inflammatory effect of diclofenac was similar to that of dexamethasone and was most pronounced with the combined use of both agents. The damage to the corneal epithelium by diclofenac was mild and reversible.
