落屑症候群の散瞳状態および調節機能

The pupil size and state of accommodation in pseudoexfoliation syndrome

　落屑症候群の散瞳状態および調節機能について，片眼性の17例で散瞳後の瞳孔径，散瞳速度，調節時間，弛緩時間，アコモドグラム，連続近点を調べた。偽落屑性のフケ様物質を瞳孔縁に認める全例において，散瞳径と散瞳速度が抑制されていた。再現性のあつた17例中11例のアコモドグラムでは4型のアコモドタイプが最も多かった。新生血管，虹彩後癒着，散瞳後の瞳孔異常を示した症例はこの症例には含まれていない。虹彩や毛様体の平滑筋の機能が落屑症候群では影響を受けている可能性がある。

I further evaluated 17 eyes with unilateral pseudoexfoliation syndrome regarding the pupil size and the state of accommodation. Eyes with rubeosis iridis or posterior synechia were not included in the series. All the eyes showed insufficient mydriatic response after topical instillation of cycloplegic and sympathicomimetic agents. Consistent accommodographic recording was obtained in 11 eyes. These eyes showed abnormal accom-modative contractile time, relaxation time and/or accommodative pattern. Type 4 was most frequently observed. The findings suggest disturbed function of iris and ciliary muscles in pseudoexfoliation syndrome.
