白内障術後のネパフェナク点眼が角膜上皮に及ぼす影響

Effects of nepafenac ophthalmic solution on the corneal epithelium after cataract surgery

要約　目的：ネパフェナク（NF），ジクロフェナクナトリウム（DF）の2種類の非ステロイド性抗炎症点眼薬による白内障術後の角膜上皮障害につきレトロスペクティブに比較検討した報告。

対象および方法：対象は2014年8月〜2015年8月に超音波乳化吸引術および眼内レンズ挿入術を施行された65例65眼。術後1週間，1か月での角膜上皮障害スコアを比較検討した。

結果：術後1週間，1か月の角膜上皮障害スコアはNF群（29眼）で0.4±0.9，0.6±1.0，DF群（36眼）で1.1±1.3，1.3±1.4でNF群が有意に少なかった。観察期間中に角膜上皮障害を発生した割合はNF群31.0％，DF群61.1％であり，NF群で角膜上皮障害の発生率が低かった。

結論：NFに比べDFで白内障術後の角膜上皮障害が強かった。

Abstract　Purpose: To report the the effect of nepafenac diclofenac ophthalmic solution on the corneal epithelium after cataract surgery.

Cases and Method: This study was made on 65 eyes of 65 patients who received phacoemulsification-aspiration followed by intraocular lens insertion during the past 13 months. After surgery, 29 eyes received instillation of nepafenac and 36 eyes received diclofenac. The corneal epithelium was evaluated on day 7 and 28 after surgery regarding superficial punctate keratopathy（SPK）using a scored system.

Results: SPK score was lower in nepafenac than diclofenac group both on day 7 and 28. The difference was significant. SPK developed in 9 eyes（31.0％）that received nepafenac and in 22 eyes（61.1％）that received diclofenac during the period of observation.

Conclusion: Corneal epithelium disorder after cataract surgery was less frequent following instillation of nepafenac than diclofenac.
