A major sarin incident occurred in the subway system in Tokyo on March 20, 1995. More than 5,000 persons were injured with lethal outcome in 11 people. We treated 33 affected persons immediately after the incident. All were diagnosed as apparently normal systemically. Severe miosis was present in 17.A 49-year-old male, who was one of the 17 victims with miosis, developed persistent conjunctival hyperemia.Bilateral symblepharon occurred 3 months later. The nasal portion of bulbar conjunctiva showed severer symblepharon and rose bengal staining. Toxic necrosis of the conjunctiva was suspected as the cause.
