ビンクリスチン投与後に視神経障害をきたした1例

A case of optic neuropathy following systemic vincristine

　64歳男性が臀部の悪性黒色腫の切除の後，ダカルバジン，塩酸ニムスチン，ビンクリスチンの全身投与を受けた。その22か月後に両眼に霧視が生じ，ぶどう膜炎と緑内障と診断された。その3か月後の当科受診時の矯正視力は右0.5，左0.3であった。その直後に前回と同じ3者併用による全身治療を開始した。その1週後に視力低下が生じ，さらに2週後には左右眼共に視力が20cm手動弁に低下していた。このとき，前房と硝子体混濁，視神経の所見には変化はなかった。右眼に盲点の拡大，左眼に視野狭窄があった。3か月後には，視力は光覚弁になり，視神経萎縮が出現した。経過中，ステロイド薬の全身投与は無効であった。この視神経障害は，細胞抑制剤，特にビンクリスチンの副作用によると思われた。

A 64-year-old male underwent surgical excision in the buttocks for malignant melanoma. He was then treated by systemic cytostatic agents including vincristine. He developed hypertensive uveitis 22 months later. Another course with systemic cytostatics was started. Visual impairment rapidly progressed until the visual acuity in either eye decreased to hand motion 3 weeks after start of systemic treatment.Visual field studies showed enlarged blind spot in the right eye and constriction in the left. Another 9 weeks later, the visual acuity decreased to light perception in either eye apparently due to optic atrophy. The optic nerve impairment appeared to be due to systemic cytostatics, particularly vincristine.
