We retrospectively evaluated the surgical results in 44 patients with primary inferior oblique muscle overaction. Weakening surgery on unilateral inferior oblique was performed in 26 patients, equal symmetric weakening surgery on bilateral inferior oblique in 10 patients, and unequal asymmetric weaken-ing surgery on bilateral inferior oblique in 8 patients. After surgery, all the eyes showed improvements in inferior oblique muscle overaction. After unilateral inferior oblique weakening surgery, 14 patients, 54%, developed contralateral inferior oblique muscle overaction. This tendency was pronounced in patients who showed more excellent surgical effect. We advocate unequal bilateral inferior oblique weakening surgery for this disease entity.
