上斜筋の形態異常と下斜筋減弱術の効果の関係

Relation between morphological abnormality of superior oblique muscle and effect of inferior oblique myectomy

（B6-2-18）　片眼先天性上斜筋麻痺患者における下斜筋部分切除術の効果を，全身麻酔下上斜筋牽引試験およびMRIによる上斜筋筋腹との関連から検討した。上斜筋牽引試験が正常，かつMRIで上斜筋筋腹の萎縮がみられない症例の多くで，下斜筋部分切除術単独で良好な結果が得られた。これに対し，上斜筋の筋腹が健側に比べて明らかに萎縮しているものは，一度の下斜筋切除術のみで満足のいく結果が得られることは少なく，初回手術から，より適切な手術方法を考慮するべきと思われた。

We reviewed 23 cases who underwent inferior oblique myectomy for unilateral superior oblique palsy. We paid particular attention to the traction test during general anesthesia and the volume of superior oblique muscle by magnetic resonance imaging (MRI). Surgery was effective in the majority of cases with normal traction force and normal volume of the muscle. Cases with small muscle responded less favorably to inferior oblique myectomy alone and neces-sitated additional procedures.
