網脈絡膜血管病変を伴った先天性アンチトロンビンⅢ欠損症の1例

A case of congenital antithrombin Ⅲ deficiency with chorioretinal vascular disorders

　多臓器に血栓症を生じた先天性アンチトロンビンⅢ（AT　Ⅲ）欠損症の39歳，男性にインドシアニングリーン螢光眼底造影（IA）を施行した。眼底検査で両眼の網膜動脈の蛇行があった。フルオレセイン螢光眼底検査では，網膜動脈の蛇行以外に閉塞性および虚血性疾患を疑わせる所見はなかった。IAにより，脈絡膜の比較的太い血管の途絶と脈絡膜循環不全を疑わせる扇状の低螢光の所見が得られ，先天性AT　Ⅲ欠損症の脈絡膜循環動態の検索にIAが有用であった。

A 39-year-old male with congenital antithrombin Ⅲ deficiency developed thrombotic lesions in multiple organs. Funduscopy and fluorescein angiography showed tortuous retinal vessels. Vasoocclusive retinal lesions were absent. Indocyanine green angiography showed occlusion of major choroidal vessels and areas of hypofluorescence suggesting impaired choroidal circulation.
