ガンシクロビルとレーザー光凝固を併用して有効であったサイトメガロウイルス網膜炎

A case of cytomegalovirus retinitis treated with systemic ganciclovir and laser photocoagulation

　症例は21歳男性，血友病を伴うhuman immunodeficiency virus （HIV）抗体陽性患者で，後天性免疫不全症候群（acquired immunodeficiency syndrome：AIDS）発症による日和見感染のため，1994年11月左眼にサイトメガロウイルス（cytomegalovirus：CMV）網膜炎を発症した。ガンシクロビル点滴療法を行ったが，副作用の骨髄抑制が生じたため投与量を減量したところ，CMV網膜炎の悪化による硝子体出血が生じ30cm手動弁となった。その後，右眼に発症したCMV網膜炎に対してガンシクロビル点滴療法とレーザー光凝固の併用を試みた。適切な時期にレーザー光凝固を行えば硝子体出血を予防でき，有効であると考えられた。

A 21-year-old male developed cytomegalovirus retinitis in the left eye. He had hemophilia and had positive antibody for human immunodeficiency virus. Vitreous hemorrhage developed after reducing the dosis of systemic ganciclovir for bone marrow suppression. Cytomegalovirus retinitis developed later in the right eye. He was then successfully treated with systemic ganciclovir and laser photocoagulation. It appeared that laser photocoagulation was effective in preventing vitreous hemorrhage.
