エキシマレーザー治療が奏効したshield ulcerの1例

Excimer laser treatment was effective in a case of shield ulcer

　春季カタルに合併したshield ulcerに対して，エキシマレーザーによる表層角膜切除を行い，良好な経過をたどった1例を経験した。症例は28歳の男性で，右眼の白内障の術後に生じた遷延性角膜上皮欠損に対し，治療用ソフトコンタクトレンズの装用などの保存的治療を行ったが改善なく，上皮欠損はshield ulcerへと移行した。当院にて上記の治療を行ったところ，術翌日から上皮の伸展，欠損部の縮小がみられ，術7日後には角膜上皮欠損は消失した。エキシマレーザー治療は，shield ulcerに対して有効な治療法の1つになりうると考えられた。

A 28-year-old male had been suffering from atopic dermatitis and vernal keratoconjunctivitis since childhood. Following cataract surgery in the right eye, corneal epithelial defect developed. It persisted despite medical treatments and developed into shield ulcer 2 months later. After treatment by photother-apeutic keratotomy with excimer laser, the lesion became completely re-epithelized 7 days later. This case illustrates the potential benefit of excimer laser surgery for shield ulcer.
