小瞳孔例の超音波白内障手術における瞳孔括約筋楔状切除による簡便な瞳孔拡張手技

Simple pupil dilatation by wedge-shaped sphincterectomy for small pupil phacoemulsification

　小瞳孔例の白内障手術における瞳孔拡張手技として，これまで困難であった上方の瞳孔括約筋切除を簡便に行う手術手技を考案した。

　本手技は，従来から報告されているような，虹彩リトラクターや硝子体剪刀などの特殊な器具を必要とせず，小切開創や長い強角膜弁を有する創からでも，上方の瞳孔括約筋切除を極めて容易に行える。本手技の導入により，全幅虹彩切開と再縫合のような，煩雑で侵襲の強い手技を用いずに，瞳孔の拡大と円形瞳孔の保持を得ることができ，従来より虹彩切開術の欠点と考えられていた，術後の瞳孔不整も軽減できた。

We devised a simple surgical technique to enlarge the small pupil during cataract surgery by phacoemulsification. The pupil sphincter is excised by superior wedge-shaped sphincterectomy. It does not necessitate special instruments as iris retractors and can also be performed in eyes with small incision or long sclerocorneal flap. This technique can be performed with ease and results in circular pupil during and after surgery. It is therefore to be preferred to conventional iridectomy in small-pupil cataract surgery.
