桐沢型ぶどう膜炎の赤外螢光眼底造影所見

Indocyanine green angiographic findings of Kirisawa-type uveitis

　桐沢型ぶどう膜炎の5症例に対して，赤外螢光眼底造影を行った。いずれも硝子体手術を施行しており，術後視力は3症例で向上，1症例で不変，1症例は光覚弁にまで低下した。従来の螢光眼底造影では，周辺部網膜の黄白色病変部は顆粒状および均一な過螢光を示した。網膜動脈の一部では血管炎による螢光色素の漏出を認めた。赤外螢光眼底造影では，黄白色病変部は低螢光を示し，その中に不整な脈絡膜血管が造影されていた。これは脈絡膜循環障害を示すものと考えた。ときに，網膜血管に一致した不連続な過螢光がみられた。これは臨床症状の悪化する時期と一致していた。

Five cases of Kirisawa-type uveitis were examined by fluorescein and indocyanine green (ICG) angiography. All cases were treated by vitrectomy and by systemic acyclovir and corticosteroids. Fluore-scein angiography showed dye leakage from retinal vessels and hyperfluorescence corresponding to white retinal lesions in the periphery. Abnormal choroidal vessels were seen, by ICG angiography, posterior to the hypofluorescent lesions, suggesting disturbed choroidal circulation. ICG angiography showed hyperfluore-scence corresponding to the retinal vessels. This feature became more manifest during periods of exacerba-tion.
