白血病治療中の小児に発症したサイトメガロウイルス網膜炎

Cytomegalovirus retinitis in a child under treatment for leukemia

　5歳男児に発症したサイトメガロウイルス（CMV）網膜炎を経験した。患児は1991年に急性リンパ性白血病と診断され，化学療法により完全寛解し，その後維持療法を受けていた。1992年7月末，不明熱で入院，9月末には尿，咽頭ぬぐい液からCMVが分離され，血液検査でCMV-lgM陽性，CMV-DNAが検出され，CMV感染症と診断された。1992年10月には左眼網膜鼻側周辺部に微細顆粒状滲出斑をみとめた。1993年1月には，出血，滲出斑，網膜血管炎，網膜浮腫を伴い典型的な病像を示した。Ganciclovirの投与をおこなったが，炎症は再燃と緩解を繰り返し，1993年6月には，右眼黄斑部にも滲出斑が出現し，視力は0.3に低下した．この時点で小児科医の判断により化学療法を中止，その後網膜炎はいったん完全に沈静化した。化学療法中止9か月後，白血病が再発，化学療法を再開し，現在も経過観察しているが，現在のところ網膜炎の再燃はない。これらの経過中，血中natural killer細胞の活性値の推移と網膜炎の推移は逆の相関関係を示した。

A 5-year-old boy had been suffering from acute lymphocytic leukemia since 18 months before. The leukemia subsided by chemotherapy and radiation. He was now under maintenance dosis of chemother-apy. One month before, he proved positive for cytomegalovirus (CMV) and was now treated with systemic ganciclovir. He was referred to us for ophthalmological examinations. Initial studies revealed retinal exudates scattered in the periphery in the left eye. Signs of anterior uveitis with retinal exudates and vasculitis developed 3 month later in both eyes. There were three episodes of CMV retinitis until apparent complete remission 6 months later.
