　メチシリン耐性黄色ブドウ球菌（MRSA）の眼感染症の1症例に対して，バンコマイシン点眼液を作成して加療した。症例は81歳女性，右眼の慢性涙嚢炎と表層角膜炎を認め，涙嚢洗浄で逆流した膿からMRSAが単独に検出された。患者本人の訴えが軽微で，高齢であることから，手術的治療は行わず，感染に対するコントロールのみを目標とした。薬剤感受性検査で，当院に採用されている点眼薬にはすべて耐性を示したため，バンコマイシンの点眼液を作成して治療した。点眼開始後2週間めにはなお微量のMRSAが検出されたが，その後は点眼を中止した後にも検出されず，当初の目標は達成された。

An 81-year-old woman presented chronic dacryocystitis in her right eye. Initial treatment with topical ofloxacin solution was unsatisfactory. Culture of purulent discharge from the affected lacrimal sac showed methicillin-resistant Staphylococcus aureus, MRSA, which was also resistant to commercially available topical antibiotics. As she declined surgical intervention, we prepared vancomycin hydrochloride ophthalmic solution, 50 mg/ml, using phosphate-buffered artificial tears as vehicle. Clinical and bacteriologi-cal cure resulted from topical instillation 4 times daily for 2 weeks.
