We analyzed the surgical dose-response relation-ship in 71 cases of unilateral superior oblique muscle palsy. Each case was evaluated regarding 49 cardinal gaze positions within 30 degrees from the fixatign point using a synoptometer. These data served as basis for developing an empirical com-puter program, SOCALC. This program cound sim-ulate the results for six types of surgery and pro-pose the optimum surgical method and dosis for each case from preoperative ocular deviations at 49 gaze positions. The accuracy of its prediction in the primary position was estimated to be within a standard deviation of 3 degrees for oblique muscle surgeries and 1 to 2 degrees for rectus muscle surgeries.
