アカントアメーバ角膜炎の臨床経過の病期分類

Classification of clinical stages of Acanthamoeba keratitis

　アカントアメーバ角膜炎を6症例（7眼）経験し，その臨床経過に共通点を見いだした。これら6症例の共通点と文献上の症例を参考に，本疾患の臨床経過の病期分類を試みた結果，次のような分類が適切であるという結論に達した。1期（初期）→2期（成長期）→3期（完成期）→4a期（消退期）または4b期（穿孔期）→5期（瘢痕期）。アカントアメーバ角膜炎はこのように1期から5期まで分類するのが適切であるが，4期までのどの病期からでも治癒に向かう可能性を持っている。この病期分類を用いることによって，本疾患がどのような病気であるか把握しやすくなり，正しく診断を下せるようになるものと考える。

We observed 7 eyes of Acanthamoeba keratitis and identified common features in its clinical pro-gression. With due consideration of literature, wepropose classification into 5 stages: early (stage 1), growing (2), established (3), regressive (4a), perforated (4b) and cicatricial (5). This classifica-tion is expected to contribute to improved diagno-sis and understanding of this disease entity.
