Myelodysplastic syndromeの白内障手術症例

Cataract surgery in a patient with myelodysplastic syndrome

　50歳女性，myelodysplastic syndrome（MDS）患者の白内障手術を経験した。最近注目されている血液疾患であるMDSは高齢者に多くみられ，増加傾向にある。MDSの白内障の合併頻度は多いと思われるが，白内障手術の報告は内外ともに未だみられない。血液疾患患者の内眼手術は易出血性，易感染性に対して特別な配慮を要する。今回筆者らは血液内科医と協力し，術前に好中球系前駆細胞のみを比較的選択的に増殖させる因子であるgranulocyte colony-stimulating fac-tor（G-CSF）を使用し，顆粒球数を改善させ，その機能を亢進させた。そして，十分な抗生物質を投与し，血小板輸血を行うことで免疫・凝固能を改善し，その後，両眼の超音波乳化吸引術を施行し，良好な結果を得ることができた。G-CSFの使用，および十分な抗生物質の投与などが有用であったと考えられた。

Particular attention is necessary in intraocular surgery in patients with blood dyscrasia due to liability of hemorrhage and infection. We operated with success on both cataractous eyes of a 50-year -old female with myelodysplastic syndrome. Prior to surgery, we tried to increase the neutrophil leu-kocytes by administering granulocyte colony-stim-ulating factor (G-CSF) and platelet transfusion. Systemic antibiotics was started the day before surgery. The cataract was aspirated by phacoemul-sification. We took utmost care to suppress intraoperative hemorrhage with a wet-field coagulator.
