直腸癌に重複した脈絡膜悪性黒色腫の1例

A case of choroidal malignant melanoma associated with rectal cancer

　直腸癌に脈絡膜悪性黒色腫を重複した1例を報告した。症例は73歳男性で，近医で右眼底の隆起性病変を指摘され当科に紹介された。約1年前の直腸癌手術の既往から転移性脈絡膜腫瘍を疑い放射線治療を施行したが，腫瘍は拡大し視力も低下したため眼球を摘出した。病理組織学的に腫瘍は脈絡膜悪性黒色腫であった。わが国の脈絡膜悪性黒色腫の重複癌は筆者らの例を含め12例報告されているが，癌の診断技術や治療法の進歩に伴い重複癌は増加の一途にあり，眼科においても重複癌を経験する機会は今後増えると思われる。

A 73-year-male underwent resection of rectal cancer one year before. He was referred to us for evaluation of possible metastatic choroidal tumor. We detected a remarkably elevated choroidaltumor temporal to the macula in his right eye. External beam radiation to the lesion failed to induce regression of the tumor, necessitating even-tual enucleation of the affected eye. Histopatho-logic studies led to the diagnosis of amelanotic spindle cell malignant melanoma of the choroid.
