MRアンギオグラフィが有用であった頸部内頸動脈閉塞症の1例

Magnetic resonance angiography was useful in a case of internal carotid artery occlusion

　血管新生緑内障を呈して当科を受診し，眼虚血症候群の診断のもとに内頸動脈閉塞症を疑い，MRアンギオグラフィで閉塞を確認できた症例を経験したので報告する。症例は62歳男性。左眼虹彩，隅角に新生血管を認め，眼底はいわゆるvenous stasis retinopathyを呈し，螢光眼底造影で腕脈絡膜循環時間，網膜内循環時間の遅延を認めた。前眼部および眼底の所見より眼虚血症候群と診断し，左内頸動脈閉塞症を疑った。確定診断のためMRアンギオグラフィ，頸動脈造影を施行し頸部内頸動脈の完全閉塞が確認された。眼科的に内頸動脈閉塞症が疑われる場合，MRアンギオグラフィは非侵襲的で，スクリーニングとして有用な検査法であると考える。

A 60-year-old male presented with neovascular glaucoma and venous stasis retinopathy in his left eye. Fluorescein angiography showed delays in arm -to-choroid and intraretinal circulation times in the affected eye. Magnetic resonance angiography and digital subtraction angiography showed completeocclusion of left internal carotid artery at the site of bifurcation. He was diagnosed as ocular is-chemic syndrome. Magnetic resonance angiography was thus of value in detecting internal carotid artery occlusion with ocular lesions as the sole manifestation.
