増殖糖尿病網膜症を呈したヘモクロマトーシスの1例

Proliferative retinopathy in a case of hemochromatosis-induced diabetes

　肝生検でヘモクロマトーシスと診断されていた64歳男性にみられた二次性糖尿病による増殖糖尿病網膜症の1症例を経験したので報告した。ヘモクロマトーシスの眼合併症としては，角膜への鉄沈着や二次性糖尿病による軽度の糖尿病網膜症の報告がいくつかあるが，増殖網膜症を合併したという報告はきわめてまれである。従来から，二次性糖尿病による網膜症は比較的軽度なものが多いとされていたが，本症例のように増殖網膜症を呈する可能性もあり，一次性糖尿病と同様に十分な眼科的管理が必要であると思われる。

A 64-year-old man was diagnosed as hemo-chromatosis involving the skin and the liver 17 years before. Insulin therapy was instituted for diabetes 3 years before. He was referred to us for ophthalmological examinations. The visual acuity was 0.5 in either eye. Funduscopy showed profuse neovascularization in the periphery in addition tohemorrhages and exudates in both eyes. Fluores-cein angiography showed extensive capillary nonperfusion in the periphery. The anterior seg-ment was free of iron deposits. Contrary to the hitherto accepted view that retinopathy in sec-ondary diabetes is mild in nature, the present case illustrates that severe proliferative diabetic retinopathy is also a possibility.
