We reviewed a series of 9 patients with optic neuritis who were either positive or false positive for antinuclear antibody tests. The series included 4 cases with a positive rheumatic factor, one with positive anti-DNA antibody and one with positive antiextractable nuclear antibodies. Conventional treatment with systemic prednisolone failed toimprove the visual outcome. Pulse therapy with mega-dose methylprednisolone appeared to be ben-eficial in 2 out of 5 attempted cases. All 4 eyes in 4 patients recovered full visual acuity without recourse to corticosteroid similar to idiopathic optic neuritis. We therefore failed to find concrete evidences that antinuclear antibody-positive optic neuritis is different from idiopathic one.
