原発開放隅角緑内障の治療成績—点眼療法，内服，ALT併用療法

Control of primary open angle glaucoma with various combinations of medications and argon laser trabeculoplasty

　原発開放隅角緑内障（POAG）の治療状況について，初診時に無治療，もしくは無治療として治療を開始し，3年以上12年にわたって経過観察した117例を対象として，生命表法により点眼療法，内服併用療法，さらにアルゴンレーザートラベクロプラスティー（ALT）併用での生存確率を検討した。最終生存確率は点眼療法のみで23.5±3.6％，内服併用で41.8±4.4％，ALT併用で67.4±6.4％であり，POAGの約30％は手術療法を必要とすると考えられた。眼圧コントロール（≦20mmHg）が得られる確率では，各々35％，50％，77％であった。

We reviewed the clinical course of 226 eyes of 117 cases with primary open angle glaucoma treated either by topical medication, topical and systemic medication, or with additional argon laser trabeculoplasty. The follow-up period ranged from 3 to 12 years, average 6.7±3.1 years. Calculationusing a life-table metnoa showea tne cumulative probability of control of glaucoma at 12 years to be 23.5±3.6%, 41.8±4.4% and 67.4±6.4% for each therapeutic modality. Control of intraocular pres-sure at or below 20 mmHg was obtained in 30%, 50% and 77% respectively.
