眼感染症分離菌に対するofloxacinの臨床的有用性の評価

Effectiveness of ofloxacin on bacteria pathogenic to the eye

　当院受診の眼感染症患者からの分離菌について，点眼用抗生剤ofloxacin （OFLX），to-bramycin（TOB），erythromycin（EM），cef-menoxime（CMX），sulbenicillin（SBPC）に対するMIC（minimum inhibitory concentration）を測定し，さらに三井らの判定基準^1,2）に準じ，臨床的有用性について検討した。MSSA（methicillinsensitive Staphylococcus aureus）およびMSS（methicillin sensitive Staphylococcus sp．）では5薬剤とも低いMIC値が得られた。一方，多剤耐性菌であるMRSA（methicillin resistantStphylococcus aureus）およびMRS（methicillinresistant Staphylococcus sp．）では，OFLXにおいて最も低いMIC値が得られた。Corynebacter-ium sp．ではEMで特に低いMIC値が得られ，Propionibacterium sp．ではTOBを除く他の4薬剤で低いMIC値が得られた。以上の結果の臨床的有用性評価より，OFLXが菌種間における抗菌力の低下もみられず最も広域な抗菌スペクトルを有する有用な点眼剤と思われる。

We evaluated the minimum inhibitory concentra-tion (MIC) of five antibiotics on 88 strains of microorganisms isolated from infectious eye dis-eases in our institution. We tested ofloxacin, to-bramycin, erythromycin, cefmenoxime and sul-benicillin. All the 5 antibiotics showed low MICvalues for methicillin-sensitive Staphylococcus aur-eus and St. sp. (MSSA and MSS). Ofloxacin showed the lowest MIC value for methicillin-resis-tant Staphylococcus aureus and St. sp. (MRSA and MRS). Erythromycin showed particularly low MIC for Corynebacterium sp. All the antibiotics except tobramycin showed low MIC for Propionibacterium sp. The findings show that ofloxacin possesses the widest antibacterial spectrum with least fluctua-tions for different strains.
