散弾様網脈絡膜症の2症例

Two cases of birdshot retinochoroidopathy

　最近経験した1症例と長期観察を行いえた1症例，計2症例の散弾様網脈絡膜症につきその臨床像を報告した。

　症例1は，71歳女性で，視朦感を主訴に受診した。前眼部炎症は軽度で，眼底には後極部を中心に1／5〜1乳頭径の黄白色の円形病巣がみられた。螢光眼底造影では，病巣に一致して過螢光が認められた。症例2は，51歳の女性で，視朦感を主訴に受診した。初診時，軽度の硝子体混濁のみであったが，8か月後に左眼眼底に黄白色病変が出現した。併発白内障が進行したため6年後に両眼手術を行った。術後，両眼底に多数の黄白色の病変が散在していた。螢光眼底造影では，病巣により低螢光あるいは過螢光の混在を認めた。視力予後は，2症例とも良好であった。全身検査，血液，生化学検査には異常はなかった。HLA-A29は症例2のみ検索を行ったが陰性であった。

We observed birdshot retinochoroidopathy in 2 females, aged 71 and 51 years each. The condition was bilateral in the first case and unilateral in the second. The first case showed classifical fundu-scopic and fluorescein angiographic features of the disease in the cicatricial stage. In the second, vitre-ous cells were the sole clinical findings. Yellowish white retinal lesions were detected in the left eye 8 months later. Depigmented splotches in the deeper retinal layers were distributed over the whole fun-dus in both eyes 6 years later. She showed negative A 29 in HLA testing. Final visual acuity was good in both cases.
