発症早期からのガンシクロビル投与が著効を奏したサイトメガロウイルス網膜炎の1例

Systemic ganciclovir was effective in a case of early stage cytomegalovirus retinitis

　発症早期からガンシクロビルの投与を行い，良好な視力予後を得たサイトメガロウイルス（以下CMV）網膜炎の1例を報告した。症例は26歳の女性で，悪性リンパ腫と全身性CMV感染症で内科入院中に飛蚊症を主訴に眼科を受診した。矯正視力は右1.0，左0.9，両眼眼底中間周辺部から周辺部にかけて，網膜動静脈の著明な白鞘化と出血および滲出斑を認めたためCMV網膜炎と診断し，ただちにガンシクロビルの投与を開始した。投与開始により網膜病変は広範な萎縮を残して改善し，初診6か月後の現在，視力は右0.6，左0.8で良好に維持されているが，副作用として白血球減少が認められた。

A 26-year-old female presented with myodesop-sia as chief complaint. She had been under treat-ment for malignant lymphoma and generalized cytomegalovirus infection. Ophthalmoscopy showed sheathing of retinal vessels, hemorrhages and exudative patches in the midperipheral and peripheral fundus in both eyes. She was diagnosed as cytomegalovirus retinitis. Systemic therapy with ganciclovir resulted in prompt resolution of retinal lesions and maintenance of favorable visual acuity in both eyes. Leukopenia was the sole adverse reaction of the treatment.
