角膜全層移植と上皮形成術を同時に行った症例

Keratoplasty with keratoepithelioplasty in a case of postraumatic corneal opacity

　角膜外傷後約30年目にデスメ膜瘤と広範な偽翼状片を伴った角膜混濁例に対し，全層角膜移植と角膜上皮移植の同時手術を行い，視機能の改善を得た。症例は58歳男性である。約30年前，ペーパーナイフの研磨中ナイフの破片が右眼に当たった。その後視力低下，眼痛が出現したため当科を受診した。初診時，右眼視力は0.06であった。右眼前眼部には広範な偽翼状片とデスメ膜瘤を認めたため，全層角膜移植と角膜上皮移植の同時手術を施行した。術後3度の拒絶反応を認めたが，ステロイド剤の点眼，内服治療で軽快し，1年後に視力は0.6に改善した。角膜血管新生を伴う角膜混濁例に対し，角膜上皮移植の併用は有効な治療手段であると考えられる。

A 58-year-old male presented with corneal opac-ity with Descemetocele and pseudopterygium in his right eye. The visual acuity was 0.06. He had suffer-ed from penetrating corneal injury 30 years before. We performed penetrating keratoplasty and ker-atoepithelioplasty. Mild rejection reaction recurredthree times after surgery and had to be treated with corticosteroid. The graft was clear one year after surgery with visual acuity of 0.6. Our case illus-trates that penetrating keratoplasty with keratoe-pithelioplasty may be useful for corneal opacity with corneal neovascularization.
