ミコナゾールの硝子体内投与と硝子体切除術が著効を呈した内因性Candida albicans眼内炎の1例

Vitrectomy with intravitreal miconazole was effective in a case of endogenous endophthalmitis due to Candida albicans

　経静脈高カロリー輸液（IVH）装着と汎腹膜炎に対する抗生剤投与を受けた23歳男性の右眼に内因性Candida albicans眼内炎が発症した。これに対してミコナゾール（MCZ）10μg／mlおよびネチルマイシン（NTL）20μg／mlを含む眼内灌流液を用いた硝子体切除術を第7病日に実施し，さらにMCZの長期全身投与を併用した。良好な視機能回復が得られ術後少なくとも11ヵ月にわたり，検眼鏡的眼底所見および電気生理学的所見では，MCZおよびNTLに起因すると思われる障害はみられなかった。よって本症に対する上記の治療法は有用な方法のひとつであると考えられる。

A 23-year-old male underwent splenectomy for portal hypertension, with intravenous hyper-alimentation (IVH) after surgery. He developed blurring of vision 4 months later. We diagnosed thecondition as fungus-induced endogenous endophth-almitis and performed vitrectomy with intravitreal injection of miconazole, 10 μg/ml, and netilimicin, 20 μg/ml. We also put him under longterm systemic miconazole therapy at the daily dosis of 600 to 1,000 mg, The outcome was favorable. No ocular tox- icity was observed for 11 months of follow-up concerning electroretinogram, visually evoked potential, slitlamp and funduscopic findings.
